To Register, simply follow the steps below:
Step 1: Collect a registration form at any Chelis office nationwide or click on the download link below.
Step 2: Fill out the information panels on the form, sign and return back to the Chelis office or scan and email to support@chelisbookazine.com. Kindly note that your application must be accompanied with a copy of your CAC form or any two of the following source documents:
- Call / Complimentary Card

Letter Head

Photocopy of company's invoice or receipts.

Company's registration number stated in the form.
Step 3: On approval, you would be sent a Unique Distributors Number. With this number, you would be identified in any Chelis office across the country, so keep this number secured.
